Important forms for your piercing appointment — please fill out in advance and bring along
A signed consent form is required before every piercing. This form serves your safety and informs you about the procedure, possible risks and the required aftercare. Please read the form carefully and bring it filled out and signed to your appointment.
For minors (16-17 years), the signature and personal accompaniment of a legal guardian is additionally required.
Adult Consent Form
18 years and older
Standard consent form for adults. Please fill out completely and bring signed to your appointment.
Minor Consent Form
16-17 years, with legal guardian
Consent form for minors aged 16 and above. Must be signed by a legal guardian who must be personally present at the appointment.
Age Policy

| 18 years and older | All piercings available without restriction. Own signed consent form and valid ID (identity card or passport) required. |
| 16-17 years | Piercings possible with written consent from a legal guardian AND their personal accompaniment to the appointment. ID of both the guardian and the minor required. Intimate piercings are excluded. |
| Under 16 years | No piercings are performed — without exception. |
| Intimate piercings | Available exclusively from 18 years of age — not even with parental consent for those under 18. |
What to Bring to Your Appointment
- Completed and signed consent form
- Valid photo ID (identity card or passport)
- For minors: ID of the legal guardian (must be personally present)
- Something to eat and drink for afterwards (helps prevent dizziness)
Tip
The consent form can also be filled out directly at the studio. To avoid waiting time, however, we recommend downloading it in advance and bringing it filled out.
