The foreskin piercing is exclusively possible for uncircumcised men, as the foreskin (prepuce) forms the tissue to be pierced. For circumcised men, this piercing is anatomically not feasible. Alternative piercings such as the frenum or dydoe may be an option.

What is the foreskin piercing?

The foreskin piercing pierces the foreskin (prepuce) of the penis with a ring or barbell. The needle is guided through the double-layered skin fold of the foreskin, so that the jewellery traverses the foreskin and is visible on both sides — outside and inside.

It is a comparatively simple and uncomplicated piercing with fast healing. The foreskin is thin, well-vascularised tissue that regenerates quickly. The procedure itself is swift and at XS Piercing completely painless thanks to local anaesthesia.

The foreskin piercing can be placed at various positions on the foreskin — on the side, on the upper or lower surface. It can be worn as a single piercing or as multiple piercings in symmetrical arrangement around the foreskin.

Oetang — Traditional roots

The name "Oetang" (also spelled "Utang") originates from Southeast Asia, where the foreskin piercing has a centuries-old tradition. Particularly among the peoples of Borneo, the Philippines and parts of Indonesia, piercing the foreskin with a small pin or ring was a widespread practice.

Traditionally, various materials were used — from bamboo to bone to metal. In modern piercing practice, we of course use exclusively implant-grade titanium (ASTM F136) or, on request, high-quality precious metals.

The cultural significance of the Oetang varies: In some cultures, it was considered a sign of masculinity; in others, it served to enhance the sexual satisfaction of the partner or had ritual significance. In Western piercing culture, the foreskin piercing is primarily aesthetically motivated, although some wearers also report a sensory component.

Variants and placement

The foreskin piercing offers various placement options:

Single piercing

A single ring or barbell through the foreskin — typically placed on the side or on the upper surface. The simplest and quickest variant, ideal for beginners.

Symmetrical pair

Two piercings placed symmetrically on opposite sides of the foreskin. Creates a balanced, aesthetically pleasing appearance and is frequently worn with rings.

Multiple / Row

Three or more piercings distributed around the foreskin. Visually reminiscent of the traditional Southeast Asian practice. Requires sufficient foreskin tissue and a step-by-step approach.

Aesthetics and stimulation

The foreskin piercing is primarily an aesthetic piercing. A ring running through the foreskin creates a striking and visually appealing appearance — especially when multiple rings are worn in symmetrical arrangement.

Some wearers additionally report a subtle enhancement of stimulation: The jewellery moves with the foreskin and can produce slight additional stimulation when gliding over the glans. This effect varies individually and depends on the placement and type of jewellery.

In daily life, the foreskin piercing is generally not visible under clothing. Depending on jewellery size, it may be slightly noticeable under very tight clothing, but is described by most wearers as unobtrusive and comfortable.

Healing and aftercare

The foreskin piercing heals comparatively quickly — the average healing time is 6 to 10 weeks. The foreskin is thin, well-vascularised tissue that regenerates efficiently.

  • Cleaning: Clean daily with sterile saline solution (0.9% NaCl). Pay attention to both the outer and inner side of the piercing channel. Gently retract the foreskin and carefully move the jewellery to loosen crusts.
  • Hygiene: Regular intimate hygiene is particularly important, as the foreskin creates a moist environment. Daily washing with lukewarm water is recommended.
  • Sexual abstinence: Refrain from sexual activities for at least 3 to 4 weeks. The jewellery in the foreskin is exposed to mechanical stress during sexual activity.
  • Leave jewellery in: Do not independently change or remove the initial jewellery during the entire healing phase.

Foreskin care

Pay particular attention to hygiene under the foreskin during healing. Smegma build-up can irritate the piercing. Daily retraction of the foreskin and gentle cleaning prevents problems.

Frequently asked questions

Does the foreskin piercing impair the function of the foreskin?

No, a correctly placed foreskin piercing does not impair foreskin function. The foreskin can still be retracted and moved back and forth as normal. The jewellery sits in the foreskin and moves with it. In rare cases, a large ring may slightly interfere when retracting — in this case, a smaller ring or barbell can help.

How many foreskin piercings can I have?

The number depends on the available foreskin area. Typically, 2 to 4 piercings are possible, distributed symmetrically around the foreskin. We recommend proceeding step by step and letting each piercing heal individually before the next is placed. During the consultation, we determine the maximum number for your individual anatomy.

Can I wear a barbell instead of a ring?

Yes, both rings (BCR, segment rings) and curved barbells are possible as jewellery. Rings are the more traditional choice and create a more striking appearance, while barbells are more discreet and less noticeable. The choice depends on your personal preferences and lifestyle.
